• Parent/Guardian & Physician Medication Administration Consent

    Medication Policy

             In accordance with PA state law, written instructions must be provided by the physician for all medication that is to be given in school, including over-the-counter medications. The instruction shall indicate the name of the child, name of medication, dosage, and time it is to be given. It must also be in the original bottle dispensed from the pharmacy. A consent form may be obtained below or from the nurse's office. The physician may use his/her own perscription pad. The parent/guardian must also give written consent to administer medication at school. All medications must be stored and dispensed in the nurse's office. Student's are not permitted to take any medication on their own. 

    Medication must be brought into the school by a parent or another adult. To ensure the safety of all of our students, please DO NOT send any kind of medication with your child!
