PHYSICAL EXAM SCHEDULE FOR 2024-2025 SCHOOL YEAR
Dr. Kenneth Sebastianelli (School Physician)will be performing the required 11th grade and sports physicals. If you prefer your own physician perform the physical exam, please have the form completed by your physician and return the required form to the nurse. DENTAL EXAM SCHEDULE 2024-2025 SCHOOL YEAR
